How can Lydia die?
I’ve read that she can only die if you kill her, not from the NPC’s in the quests.
From what I’ve seen when I played this is true.
Lydia just falls to her knees, pants then recovers if she is beaten.
However, Faendal, another follower died at the hands of another NPC..

Revive her using a console command. Open the console and click on her body. A code will appear. Simply type “resurrect” into the console, press enter, and close the console. She will be resurrected.
